Your Family Extension

Our programs are centered on promoting wellness and supporting social and physical wellbeing. Our adult day services are in a community group setting to monitor clients properly and establish independence at the same time.

Mission Statement

When loved ones begin experiencing a decrease in physical, mental, or social functioning, it can be difficult for the entire family. Family members may be able to assist at home, but busy schedules can make caregiving difficult.
